Every template marked live is part of the active rotation; the picker hashes (round_id, salt) to choose templates per round, so each player sees a fresh sequence from the same set. To add a new template you edit internal/minigames/mentalmath/templates.go and redeploy — there is no admin "add new question" form because there are no per-day question rows. Pool-health runway is infinite for this game.
